Family Nurse Practitioner (FNP)
Baton Rouge Primary Care Collaborative (BRPCC) is seeking an experienced and compassionate Family Nurse Practitioner (FNP) to join our growing medical team.
The Family Nurse Practitioner provides comprehensive primary care services, including health assessments, diagnosis and treatment of acute and chronic conditions, preventive care, medication management, patient education, and coordination of care.
The ideal candidate is confident in their clinical judgment, works well as part of a collaborative healthcare team, and is committed to providing respectful, high-quality care to patients from diverse backgrounds, including individuals experiencing financial or housing instability.
Responsibilities
- Conduct comprehensive and episodic health assessments and physical examinations.
- Assess patient medical history, symptoms, developmental needs, psychosocial factors, and other information relevant to treatment and care planning.
- Diagnose and manage acute and chronic health conditions within the Nurse Practitioner's scope of practice.
- Develop and implement individualized treatment and care plans.
- Order, perform, and interpret appropriate laboratory testing, diagnostic studies, and other clinical procedures.
- Prescribe and manage medications in accordance with Louisiana requirements, applicable agreements, and BRPCC policies.
- Monitor patient response to treatment and adjust care plans as appropriate.
- Provide preventive care, health education, wellness counseling, and harm-reduction education.
- Coordinate follow-up care, referrals, and access to community resources when needed.
- Maintain accurate, timely, and complete clinical documentation.
- Collaborate with physicians, nursing staff, and other members of the healthcare team to support continuity and quality of care.
- Serve as a clinical resource to healthcare staff and contribute to staff education and development as appropriate.
- Maintain current professional knowledge and complete continuing education necessary to maintain licensure and certification.
- Participate in BRPCC quality and performance improvement activities.
- Follow applicable BRPCC policies, grant requirements, regulatory standards, and governmental guidelines.
- Identify resources needed to support patients and advocate for additional resources when appropriate.
- Participate in activities that support BRPCC's mission and the needs of the communities we serve.
- Perform other related duties consistent with the position and scope of practice as assigned.
Education & Experience
- Master of Science in Nursing (MSN) from an accredited nursing program.
- Current Louisiana licensure and certification to practice as a Nurse Practitioner.
- Current prescriptive authority as required by the State of Louisiana.
- Current Family Nurse Practitioner certification.
- Minimum of 2 years of Family Nurse Practitioner experience .
- Strong knowledge of physical assessment, differential diagnosis, pathophysiology, pharmacology, and the management of acute and chronic conditions.
- Strong clinical judgment and ability to independently assess patients and develop appropriate treatment plans.
- Strong interpersonal and communication skills with patients, families, and healthcare team members.
- Experience working in primary care, community health, or a similar clinical environment preferred.
- Comfortable providing care to diverse patient populations, including patients who may be experiencing homelessness and/or financial hardship.
- Must be authorized to work in the United States.
