OREGON INTEGRATED HEALTH
Oregon Integrated Health is an Integrated Primary Care Medical Home hiring a part-time Registered Nurse for our Florence clinic, a designated Rural Health Clinic (RHC) serving the central Oregon coast. We are a Tier 5 Patient Centered Primary Care Medical Home and a Vaccines for Children Program participant. Our Florence team includes Family Nurse Practitioners, Naturopathic Physicians, Behavioral Health Clinicians, Medical Assistants, and a Community Health Worker, with telemedicine connecting patients to providers across our four Oregon locations (Portland, Eugene, Salem, Florence).
- JOB DETAILS
- FULL-time, non-management RN — no direct reports; a patient-facing clinical position
- Flexible with hours and scheduling — days and hours set with you, within clinic operating times
- $3,000 Sign-On Bonus, paid first pay period following 90 days of employment with OIH
- Structured onboarding: trained and mentored by our Lead RN with written workflows and training materials for every core function — new graduates and experienced RNs equally encouraged to apply
- JOB DUTIES AND RESPONSIBILITIES
- Chronic Care Management (CCM): manage an assigned panel of enrolled CCM patients — monthly outreach, care plan updates, time tracking, and care coordination
- Medicare Annual Wellness Visits (AWV): conduct AWVs including health risk assessments, screenings, and personalized prevention plans
- Clinical Triage: provide telephone and in-clinic triage using standing orders and established protocols; route patients to the appropriate level of care
- VFC Program: maintain Vaccines for Children inventory — storage and handling, temperature logs, counts, ordering, and compliance documentation (prior VFC experience helpful; full training provided)
- Provide appropriate medical record documentation in the EHR on all patients in accordance with clinic policy
- QUALIFICATIONS, SKILLS AND ABILITIES
- Current, unencumbered State of Oregon RN license (required); BLS certification (required)
- New graduates welcome — structured training and Lead RN mentorship provided
- Previous experience in ambulatory, care coordination, or primary care settings preferred
- Comfortable and skilled with electronic platforms such as EHR
- Valid driver's license and reliable transportation
PAY AND BENEFITS
- Protected Sick Time: accrues 1 hour for every 30 hours worked, max use of 40 hours per year, available immediately as time accrues after first day of employment
- 401(k) Benefit: OIH matches 100% of contributions up to 3% of compensation, plus 50% on deferrals greater than 3% up to 5% (eligibility: 1 yr employment, 1,000 hrs worked, at least 21 yrs of age)
- Continuing Education Benefit up to $700 annually, completing 90 days of employment
