About the Role

We're looking for a Community Outreach and Care Coordination Lead to launch and run our Upstate New York field team, starting in the Finger Lakes. This is a player-coach role: you'll supervise four Community Outreach Coordinators across 14 counties while doing your own screening, navigation, and enrollment work in the field — roughly half your time on each. You'll own our highest-value regional partnerships, from health systems and FQHCs to food banks and community organizations, and serve as the local escalation point for our SCN partner. You'll report to the NY Regional Manager, Community Outreach.

Compensation & Benefits

The expected salary range for this role is $65,000 - $75,000 annually, plus performance bonus and benefits. Compensation for this role will depend on various factors, including qualifications, skills, and location.
