The Palliative Care Social Worker serves patients with serious illness and their families through advance care planning, psychosocial assessment, care coordination, and connection to community resources. This role focuses on improving quality of life and is part of our interdisciplinary team to serve patients with advanced heart failure. Qualifications: Master's Degree in Social Work and Ohio LSW required. LISW preferred. Healthcare, community or hospital social work, care management, or case management experience preferred. Position is Part-Time, Monday-Friday, no weekends or holidays, 20 hours per week. Location is at Bethesda North Hospital campus.
