The Continuous Care (CC) Licensed Practical Nurse (LPN) provides direct patient care to hospice patients experiencing acute symptom crises requiring Continuous Home Care services. The CC LPN delivers compassionate, skilled nursing care focused on symptom management, patient comfort, safety, and support for caregivers during periods of increased patient need. Care is provided in patients' homes, assisted living facilities, long-term care facilities, and other community settings.
Three 12-hour shifts per week
Day Shift: 7:00 AM to 7:30 PM
Every third weekend required
Holiday commitment per departmental guidelines
