Job Overview

Location/Schedule: This position is based at the Kerns building located in NE Portland, OR, but some work responsibilities are conducted in the community. The schedule for this position is Monday through Friday, 8:30 a.m. to 5:00 p.m. with some weekend and holiday availability required as necessary.

About the FACT Program: The Forensic Assertive Community Treatment (FACT) program is a specialized mental health service designed for individuals with Serious and Persistent Mental Illness (SPMI) who have current or historical involvement with the justice system. FACT employs a multidisciplinary, community-based approach to deliver intensive, client-centered, and time-unlimited services aimed at improving mental health outcomes and daily functioning. The program addresses criminogenic risks to reduce recidivism through comprehensive services, including individual therapy, case management, crisis support, substance use disorder counseling, supported employment, medication management, peer services, housing support, and medical coordination. This role includes participation in an after-hours, on-call rotation to support clients in crisis.

Description: The Program Supervisor II works within an interdisciplinary team to coordinate community-based services for individuals assigned to the FACT Team. This role is crucial in supporting clients through a strengths-based, client-centered, trauma-informed approach to reduce recidivism and achieve personal goals. The Program Supervisor II manages the daily operations of the program, provides clinical oversight, and ensures quality service delivery. This position requires direct clinical service provision to clients at least 50% of the time and works closely with community partners, including the Multnomah Department of Community Justice and Portland Police Bureau.

Working Conditions:

Environmental: This role is community-focused, with primary operations based at the Kerns Building and involving frequent travel to various locations for meetings and service provision; it is designed for in-person work and does not include hybrid or remote work options. The environment may be dynamic and occasionally requires the Supervisor to perform tasks of other team members to maintain program stability. Work is performed in one or more of the following: clinical office, acute care settings and community setting including client residences, correctional facilities, shelters, hospitals, public places and other social service sites. Meal periods and breaks are subject to interruption. Universal Precautions and remaining alert to the environment are critical to address the potential for violent behavior, exposure to disease, biohazards, noise, and contaminants. Must possess the ability to work some weekends, evenings, and provide on call support to assigned team as required. This role would be required to be on an after-hours on-call rotation.

Mental Demands: The work assigned is diverse and involves addressing new and unusual circumstances. The work regularly involves a degree of unpredictability and disruption of planned tasks requiring a flexible time management approach. In addition, this position requires exercising sound judgment.
